More babies are being born by caesarean section in England than delivered naturally – without assistance – for the first time.

Latest NHS data for 2024-25 shows that 45% of births were by C-section, compared to 44% which were spontaneous, a term used to describe a natural vaginal birth. Another 11% needed extra support and interventions like forceps.

Nearly half of the caesaereans – when the baby is delivered by surgeons cutting into the mother’s abdomen and womb – were planned in advance.

NHS England says the increase is influenced by many factors, including women choosing to have this kind of birth and a rise in pre-existing conditions such as diabetes and obesity.

Experts say the picture behind the figures is “very complex”.

Dr Donna Ockenden, who is chairing an independent review into maternity services at Shrewsbury and Telford Hospitals, says it has been an “evolving picture over time”.

“We know there is an increasing maternal age, we know there is rising ill health. I also think previous experience will play a part.

“Birth trauma and having a caesarian section first time, may influence their decision to have a C-section the second time round.”

For many women, having a C-section (also known as a caesarean) is a matter of choice.

Over the past 10 years, the number of caesareans has doubled.

Dr Alison Wright, president of the Royal College of Obstetricians and Gynaecologists said part of this was down to improvements “in our ability to detect when a baby’s wellbeing is deteriorating during labour, and therefore respond appropriately”.

‘Least worst option’

Soo Downe, a professor of midwifery studies at the University of Lancashire, says there are many factors contributing to the rise.

“The average age of being a mother has only gone up by one year over the past decade, and women’s BMI [Body Mass Index] has only increased very slightly.

“This doesn’t explain this very rapid rise that has happened over the last 10 years.”

She suggests part of the picture is a growing number of women who feel a caesarean is the “the least worst option” because there is a fear they might not get the support they need for a “safe straightforward positive labour”.

Maternity services in England have been heavily criticised, with reviews into maternity care ongoing at 14 NHS trusts.

An NHS spokesperson said: “All women should receive safe, personalised maternity care in line with best practice guidance and clinical evidence.

“The priority should always be the safety and wellbeing of both mothers and babies, and decisions about how a baby is born are made through informed discussions that consider individual circumstances, clinical advice and a woman’s preferences, ensuring the safest and most appropriate approach for each birth.”

A common vaginal condition known as bacterial vaginosis (BV) may actually be a sexually transmitted infection (STI), according to researchers.

The NHS currently says BV is caused by “a change in the natural balance of bacteria in your vagina and is not an STI”, even though it can be triggered by sex.

However, a study claims BV – which affects nearly a third of women worldwide and can lead to infertility, premature births and newborn deaths – is spread during intercourse and fits the definition of an STI.

The Australian trial, published in the New England Journal of Medicine, found treating sexual partners, not just the patient, can be vital to clear the infection.

What is BV?

BV is a common cause of unusual vaginal discharge that has a strong fishy smell.

The colour and consistency may change too, such as becoming greyish-white as well as thin and watery.

Half of women with bacterial vaginosis do not have any symptoms though.

Bacterial vaginosis does not usually cause any soreness or itching.

It can be treated with antibiotic tablets or gels or creams.

In the trial of 164 couples with BV, the researchers achieved higher cure rates by treating BV as an STI, with both sexual partners – rather than just the woman – given antibiotics.

The doctors stopped the study early when it became clear that BV recurrence was halved using this approach.

One of the lead researchers, Prof Catriona Bradshaw, said: “Our trial has shown that reinfection from partners is causing a lot of the BV recurrence women experience, and provides evidence that BV is in fact an STI.

“Part of the difficulty in establishing whether BV is sexually transmitted has been that we still don’t know precisely which bacteria are the cause, but advances in genomic sequencing are helping us close in on that mystery,” she added.

In the study, run by Monash University and Alfred Health researchers at the Melbourne Sexual Health Centre, half of the men got an oral antibiotic and a topical antibiotic cream to apply to the skin for one week, while the control group of men were given no treatment.

Given the findings, the clinic has now changed its clinical practice to routinely treat both partners.

The British Association for Sexual Health and HIV said the findings provided “valuable evidence supporting what has long been suspected – that BV-associated bacteria may be sexually transmitted, particularly in those who experience recurrent infections”.

“This research enhances our understanding of BV and offers promising insights that may help to guide treatment approaches in more persistent cases,” said a spokesperson.

Ms Vickery, who is from Eglinton in County Londonderry, likened her pain to “shards of glass rubbing against her organs” which was “excruciating” especially when passing urine, or even when walking.

She spent more than £20,000 to have the mesh removed which included the private surgery and travel costs to St Louis, Missouri, in the US.

Ms Vickery had vaginal mesh implants at Altnagelvin Hospital in 2014 to treat urinary stress incontinence.

In March 2023, she began to feel a burning sensation in her lower stomach and felt extreme pain, especially when passing urine.

She recalled one episode when she spent 45 minutes in her bathroom.

“There was just so much blood in the toilet bowl and I noticed I had passed a piece of the mesh which felt like it was cutting through me.

“The pain was excruciating, I felt I was going to pass out,” Ms Vickery said.

Lisa Lunt, partner at law firm Pogust Goodhead, which led the actions on behalf of the women, said those implanted with transvaginal mesh had suffered years of chronic pain and suffering.

But she said lots of women had missed out on payouts because of a deadline to claim.

“Hundreds of women were prevented from making a claim due to strict 10-year time limits that are in force from the date that the product was manufactured.

“It’s about time that the government took action to increase the time limits,” she said.

Kath Sansom, from campaign group Sling the Mesh, said that it was “groundbreaking” to achieve the successful group action, but it would only benefit a fraction of women harmed by the mesh products.

She believes 40,000 women could be affected by mesh complications – about 10 to 20% of those given mesh implants.

There has already been a successful group action in Scotland.
